For more than 30 years, we have supported action on the social determinants of health.
With a network of over 2,300 members and supporters and 140 community and voluntary organisations, we have extensive reach across Northern Ireland and beyond.
We focus on disadvantaged communities and raising awareness of the root causes of poor health and inequality.
We bring together people, communities, organisations and decision-makers to identify needs, recognise local strengths, share skills, generate evidence and take action to create a fair and equal society.

Our Vision and Mission
Our Vision
A fair and equal society where everyone experiences their best health and wellbeing
Our Mission
We champion community development approaches to tackle the social determinants of health and end health inequalities
Our Values
We believe fairness and equality. Everyone should have the same chance for a healthy life, no matter where they were born or live. We work to create conditions where people have more control of their health and wellbeing, and a voice in decisions that affect them.
We support people to have a voice, be included and shape decisions through meaningful participation.
We act with honesty, openness and accountability. We build relationships based on trust, compassion, and mutual respect, and ensure our actions reflect our values.
We value diversity, challenge racism and discrimination, and will always work to build an inclusive and equitable society.
We believe that empowered communities can create lasting change by working together to achieve health equity.
Our Strategy
CDHN’s Strategic Plan for 2025–2030 is rooted in our mission, vision and values.
The plan sets six strategic priorities which reflect our belief that communities must be central to creating solutions.
