Recognising and Reducing Health Inequalities: community development in a GP Practice multidisciplinary team

This case study explores the impact of the Elevate programme on a team of social workers who support patients in a GP practice as part of a multi-disciplinary team. The Evaluate workshop led them to consider the difference between community-based work and community development, the benefit from interacting with the community and voluntary sector, and the connections between social determinants of health, multi-disciplinary teams and connecting with the community.
